Uniform delivery ticket is a document that must be attached to every security delivered by the seller, making the security "good delivery".

Applying "Uniform Delivery Ticket" to Securities Exams:

When a seller of securities deliver physical certificates to the buyer, the securities must be in good deliverable form. This includes that the certificate must be in good condition, signed by all owners, all owners must be alive and it must be accompanied by uniform delivery ticket. The uniform delivery ticket will include the particulars of the trade including the trade date, settlement date firm name, phone number and cusip number of the security.
